Weekend Provider in North Houston– Post-Acute Care (Minimum 2 Full Weekends/Month)

Exciting Opportunity in a Physician-Led Organization

Trumen Physicians + Associates is contracting with an experienced and compassionate Nurse Practitioner to provide post-acute care services in the North Houston area on weekends. This flexible independent-contractor opportunity allows you to work any two full weekends (Saturday & Sunday) per month, delivering in-person care to patients in skilled nursing and rehabilitation facilities.

Responsibilities:

  • Provide in-person weekend care to patients in skilled nursing facilities
  • Manage acute and chronic conditions in collaboration with Trumen-affiliated physicians and facility staff
  • Perform patient assessments, update treatment plans, and review labs and diagnostics within your scope of practice
  • Communicate and collaborate with facility teams and Trumen physicians as needed
  • Document all visits and clinical decisions in the EMR

Qualifications:

  • Active Texas Nurse Practitioner license
  • Minimum 1 year of experience as an NP (post-acute/SNF/geriatric preferred)
  • Strong clinical judgment and ability to work independently
  • Excellent communication and organizational skills
  • Availability of working at least 2 full weekends per month (Saturday & Sunday)
  • Reliable transportation within the North Houston area
