Position Summary
- Assist clients in attending scheduled physical health and wellness appointments and support follow-through with treatment recommendations.
- Monitor and document client compliance with medical and wellness plans.
- Support clients in transitioning to appropriate community-based health and social service providers.
- Provide safe transportation for clients using personal vehicle in accordance with agency policies.
- Maintain accurate, timely documentation and data entry in required systems meeting agency defined standards and expectations.
- Collaborate with healthcare providers and community partners to coordinate client care.
- Educate clients on health management, appointment preparation, and self-advocacy.
- Observe and report changes in client health, functioning, or risk concerns.
- Maintain professional boundaries while building supportive, therapeutic relationships.
- Perform services in various environments including hospitals, jails, clinics, the community "meeting the client where they are."
- Perform other duties as assigned.
