• Conduct thorough psychosocial evaluations to assess clients’ mental health challenges, strengths, and available resources, ensuring a well-rounded understanding of their needs.
• Provide individual, family, and group therapy sessions using evidence-based modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care.
• Develop and implement personalized treatment plans tailored to each client’s specific goals, continually reassessing and adjusting strategies as necessary.
• Facilitate collaborative care by partnering with psychiatrists and other healthcare professionals, ensuring integrated and effective treatment initiatives.
• Deliver crisis intervention and risk assessments as needed, crafting safety plans to support clients in immediate distress.
• Lead psychoeducation programs and support groups that empower clients through shared experiences and knowledge.
• Maintain accurate and up-to-date documentation in compliance with ethical standards and organizational policies.

In this role, you will work with a diverse clientele that includes individuals coping with the mental health impacts associated with terminal illness and significant life transitions. Your expertise will be crucial in understanding and addressing issues such as grief, anxiety, and depression within the context of hospice care. The focus is not only on the individuals receiving care but also on their families, ensuring a holistic approach to emotional and psychological health at this critical time.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a mental health or outpatient setting, demonstrating a solid understanding of therapeutic practices.
• Strong interpersonal communication and assessment skills that foster a trusting environment for clients.
• Commitment to utilizing evidence-based therapy techniques to ensure the best outcomes for clients.

• Prior experience working in hospice, palliative care, or related fields is highly desirable.
