Job description

The Director of Therapeutic Programs is responsible for creating, implementing and leading therapeutic activities and groups that cater to the unique residents’ needs at Affinity Healthcare, LLC.

To interact directly with residents, train behavioral health staff and participate in the management of the resident experience.

Our resident population include medically complex neurological disorders and behavioral health disorders. 

The role includes directing the day-to-day operations of the behavioral program and the implementation of programmatic and administrative policies and procedures in accordance with all applicable regulatory agencies and state and federal requirements for SNF level of care. Assumes 24 hour on call responsibility for notification of and resource for all administrative and clinical issues.

  • Managing a team of paraprofessional engagement specialists/mental health

  • Create comprehensive treatment plans in collaboration with the clinical team members

  • Monitoring the quality of care provided by the behavioral health team

  • Implementing training programs for staff to improve their therapeutic skills and knowledge

  • The Director is integrally involved with the admission and discharge processes of residents including development of plans of care and communicating with referral sources, families, funders, and prospective discharge sites.

  • Assessing the effectiveness of treatment plans and making necessary adjustments

  • Conducting regular staff meetings to discuss patients’ progress, facility needs, and department goals

  • Managing the department budget and seeking funding opportunities when necessary

  • Maintaining accurate records of patients’ treatment progress
