New Growth at New Dimensions! We are expanding our services and are seeking a Registered Nurse (RN) to join our home health team in the Fergus Falls area. This is a great opportunity for a motivated RN who values independence, flexibility, and providing high-quality, person-centered care.

In this role, you will have the opportunity to build your own schedule while working directly with clients in their homes, helping them achieve the best possible outcomes in a comfortable, familiar environment.

Training is provided for the right candidate.

What You’ll Do

  • Perform INR draws and medication management support
  • Set up and assist with medication organization
  • Serve as a liaison between clients and physicians
  • Complete patient assessments and accurate documentation
  • Supervise Personal Care Assistants (PCAs)
  • Support coordinated, patient-centered care plans
  • Ensure continuity of care and communication across providers
