About the Role

This LPN role isprimarily based in our Rotterdam PACE Medical Clinic, withoccasional community visitsfor medication support, wound care, lab draws, or other followup needs. You'll provide skilled nursing care to older adults enrolled in the PACE program-helping them remain safely at home while avoiding unnecessary hospitalizations or nursing home placement.

You will work closely with:

  • Physicians
  • RN Case Managers
  • Social Workers
  • Therapists and Home Care staff

What You'll Do DaytoDay

In the Clinic

  • Room participants and prepare them for provider visits
  • Administer medications, injections, vaccines, and treatments
  • Perform vital signs, documentation, and EMR charting
  • Assist with procedures while ensuring comfort and dignity
  • Manage clinic medications, supplies, and infection control
  • Support phlebotomy, specimen collection, INR checks, and testing

In the Community (Occasional)

  • Home visits for mediset changes, wound care, labs, or medication followup
  • Deliver medications or supplies when needed
  • Communicate changes to the RN Case Manager promptly

Pay Range: $24.00 - $30.10
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
