Primary Purpose
Advantmedย isย seekingย a dedicated and compassionate Nurse Practitioner (NP) to join our full-time provider network. In this role, you will performย in-homeย wellness and risk adjustment assessments for diverse patient populations with varying healthcare needs. Your work will ensureย accurate, comprehensive documentation and support high-quality,ย value-basedย clinical care across the state.ย
Key Responsibilitiesย
In-homeย Clinical Assessmentsย
- Travel to membersโ homes and conduct comprehensive health evaluationsย ย
- Conduct Annual Wellness Visits and health assessments for members with chronic conditions.ย
- Medical history and physical examinationsย
- Medication review and reconciliationย
- Possible preventive health screeningsย
- KED (Kidney Health Evaluation)ย
- DRE (Diabetic Retinal Exam)ย
Patient Education & Engagementย
- Deliver preventive health education and counseling opportunities.ย
- Educate patients on ongoing health monitoring, medication adherence, and chronic condition management.ย
- Support informed decision-making and ensure true, informed consent.ย
- Engage in goalsย ofย care discussions whenย appropriate.ย
Care Coordination & Quality Supportย
- Identifyย clinically relevant diagnoses for care management and risk adjustment documentation.ย
- Communicate findings used to generate postย visit summaries for the primary care provider.ย
- Identifyย and help close quality care gaps (e.g., screenings, labs, follow-ups).ย
- Collaborate enthusiastically with the broader care team to enhance clinical quality and patient experience.ย
Travel Requirementsย
- Full-time providers support the entire state of Oregon.ย
- Daily travel expectationsย remainย within a 55-mile radius of the assigned home location.ย
- Statewide travel will be scheduled with a minimum of two weeksโ notice.ย
Compliance & Professional Standardsย
- Adhere to HIPAA regulations and safeguard protected health information (PHI).ย
- Maintainย accurate,ย timely, and compliant EMR documentation.ย
- Performย additionalย duties as assigned in alignment with organizational goals.ย
Requirements
Qualifications
- Active, unencumbered Nurse Practitioner (NP) license in Oregon.ย
- Minimum 3 years of direct patient care experience (primary care, adult/geriatric preferred).ย
- EMRย proficiencyย is desired.ย
- Priorย in-homeย risk assessment experience is a plus but notย required.ย
- Bilingualย proficiencyย is an advantage.ย
