Job description

Overview

Community Care Coordinator - C.N.A. or LPN - Wellness Home - $6,000 Sign-On Bonus (to be paid throughout the first year of employment).

This full-time CNA or LPN position is with LIFE Butler County for the Wellness Home.

Hours: 6:30 a.m. to 3:00 p.m. 

Lutheran SeniorLife provides a comprehensive array of community-based health and social service programs throughout western Pennsylvania.

Our services include LIFE (Living Independence for the Elderly); skilled nursing, home health; personal care; adoption & foster care; assisted living; adult day services; memory care; hospice; palliative care; residential living; affordable housing; home safety monitoring; Meals on Wheels; rehabilitation services and private duty.

LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.

Enhances the abundant life of participants by providing assistance through performance of routine direct and indirect participant activities according to established policies and procedures. Maintains participant rights, privacy, and confidentiality. Promotes participant acceptance of care plans and responds promptly to overall participant needs.

Qualifications

Community Care Coordinator - C.N.A. or LPN requirements include:

  • High school diploma or general education degree (GED); or 1 year related experience and/or training; or equivalent combination of education and experience.
  • LPN or Current Pennsylvania State Nurse Aide Certification; Must be listed on state nurse aide registry and without allegations.
  • Must have one (1) year of geriatric related experience;
  • Working knowledge of computers, Microsoft Word and Excel required;
  • Excellent interpersonal skills, team-oriented, knowledge of medical terminology, computer literate.
  • Must have and present current valid driver's license.
  • CPR and Certified First Aide required.
