Position Summary:

The purpose of the Registered Nurse (RN) is to deliver care to residents utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation under the direction of the residents’ attending physician. The RN will effectively interact with residents, family and other health team members while maintaining all standards of professional nursing.

Responsibilities of Registered Nurse (RN):

  • Providing nursing care to patients as assigned
  • Administers, and records medications and treatments as assigned
  • Prepares for and assists with diagnostic procedures and treatments
  • Reinforces support and provides health care instructions to patients and their families
  • Participates with other team members in planning, implementing, and evaluating nursing care and other patient activities
  • Assists in the admission, transfer, and discharge of patients
  • Assists with unit management and/or assumes charge of unit as assigned

Qualifications of Registered Nurse (RN):

  • High school diploma or equivalent required
  • Graduate of an accredited school of nursing
  • Valid PA state licensure as a Registered Nurse (RN) or Graduate Nurse (GN).
  • Current AHA health care provider training (BCLS).
  • Demonstrated experience within a skilled nursing facility and ability to provide proof of such experience.
  • Valid PA RN/GN License

JOB DESCRIPTION:

  • Utilize nursing knowledge and skills in the safe implementation of preventative therapeutic and rehabilitative nursing care of assigned resident as evidenced by documentation and observation of positive resident care outcomes.
  • Performs independent nursing functions using established procedures, policies, guidelines and standards.
  • Admits and assess residents including initial care plan.
  • Administers assessments, treatments, medications, and diets accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes.
  • Completes all required resident charting every shift.
  • Makes rounds on unit every shift; assess orderliness, infection control and resident care
  • Observes and documents changes in residents’ condition and report to the attending physician and other departments.
  • Participates in discharge planning with physician, and other departments according to residents’ needs.
  • Directs, supervises, and assists CNAs and other nursing staff in providing bedside care.
  • Works collaboratively with other members of the health care team in coordination of the resident’s care as evidenced by other department feedback and coordinates communication among team.
  • Performs other duties as assigned.
  • Directs, supervises, and assists nursing assistants and others in providing direct resident care, adjusting assignments as necessary.
  • Promotes a climate conducive to cooperation of all staff to ensure the continuity and quality of resident care.
  • Admits and assesses residents including the initial care plan and all admission assessments.
  • Administers treatments, medications and diet as prescribed; ensures medications/ treatments have appropriate diagnosis
  • Interacts/Communicates well with co-workers, physicians, residents, families and visitors.

  • Participates in discharge planning with physician, and other disciplines, according to resident’s needs.
  • Accepts assignments as “Charge Nurse” when indicated by RN supervisor.
  • Practices nursing according to nursing theory and current standards of practice.
  • Attends scheduled in services/ meetings.
  • Follows standards of practice and infection control standards for skin treatments; observes residents for any skin changes.
  • Checks narcotic count and reconciliation sheets and signs in logbook at beginning and end of shift.
  • Completes all required resident charting every shift. (i.e.: Medicare documentation/ alert charting)
  • Educates, directs, assists in resident nursing care
  • Makes rounds, checking residents and resident rooms for cleanliness; infection control issues and ensuring residents are cared for properly.
  • Completes 24-hour report and gives thorough report to oncoming nurse. (including any calls that have been placed or need to be placed to physicians, RP’s, etc.)
  • Observes and documents changes in residents’ condition and reports changes to RN supervisor; residents’ physician and resident’s RP if needed.
  • Supervises and assists in bowel and bladder training program for residents, as needed.
  • Able to organize work and prioritize.
  • Receives medications and medical supplies daily and checks accuracy of orders received; logs narcotics appropriately; notifies RN supervisor of any discrepancies as soon as possible.
  • Communicates with direct care staff with all resident care concerns at the beginning and throughout the shift.
  • Answers families’ and visitor’s questions as necessary.
  • Is responsible for emergency equipment and general nursing supplies; restocking medication/treatment carts and any other supplies used.
  • Participates in care plan meetings when asked.
  • Makes rounds with physicians.
  • Reports all accidents and incidents to RN supervisor and documents on resident chart and on incident reports; informs doctor and family.
  • Transcribes physician’s orders as per p/p; ensures all orders are written correctly and have appropriate dx.
  • Monitors hydration of residents, including accurate I & O. Notifies dietician and MD if not meeting fluid needs or exceeding needs and documents same in residents’ chart.
  • Makes rounds during mealtime to see that each resident has received help as needed, assists in hand feeding, assures that sufficient fluids, special feedings and nourishments are administered as needed.
  • Completes assignments timely, i.e.: quarterly evaluations
  • Coordinates activities with ancillary departments to ensure that treatments plans are followed and/or revised as needed. (i.e.: therapy)
  • Utilizes equipment properly and ensures direct care staff does same; communicates to RN supervisor if education is needed.
  • Monitors weight of all residents, utilizing protocol if there is a significant weight loss.
  • Ensure plan of care is being followed.
  • Cooperates with all staff; is respectful to coworkers.
  • Practices safe working habits.
  • Answers call l lights promptly.
