BayCare is currently in search of our newest TOC Clinical Pharmacist who is passionate about providing outstanding customer service to our community.

Clinical Pharmacist Responsibilities :

The TOC Clinical Pharmacist shall support the mission of the hospital by aiming to reduce hospital readmissions through patient discharge follow-up. Perform clinical duties and engage in patient care to assist with medication safety, increase patient medication and disease state understanding, and encourage provider follow-up and continuity of care following hospital discharge.

Clinical Pharmacist position details:

  • Facility: St. Joseph's Hospital

  • Location: Tampa, FL

  • Status: PRN as needed

  • Shift: 8:00AM - 4:30PM

  • Weekend Work: None

  • On Call: No
