This position is onsite-based within a home health care setting, allowing you to provide therapeutic services directly within the comfort of your patients’ homes. You will collaborate with a dedicated healthcare team, ensuring that each patient receives coordinated care tailored to their specific rehabilitation goals.

• Conduct comprehensive patient assessments to evaluate functional ability, consider medical history, and develop personalized treatment plans aimed at achieving rehabilitation goals.

• Deliver direct patient care through individualized therapy sessions, demonstrating advanced techniques and utilizing industry-standard practices to improve patient outcomes.

• Utilize clinical skills to assess therapeutic and functional status, ensuring that each patient progresses effectively through their rehabilitation journey.

• Instruct patients, caregivers, and families on therapeutic exercises, the use of assistive devices, and ways to enhance mobility outside of structured therapy sessions to enable independent living.

• Prioritize caseload and coordinate daily assignments in collaboration with a multidisciplinary team to ensure efficient and thorough patient care.

• Maintain detailed patient documentation to track progress, communicate effectively with the care team, and adjust treatment plans as needed based on patient responsiveness.

In this role, you will work with a diverse patient population, including both adult and pediatric clients. From post-surgical rehabilitation to managing chronic conditions, you will be equipped to address various orthopedic and neurological issues, tailoring your approach to meet each patient where they are in their recovery process.

• Must hold a valid license to practice Physical Therapy in the state of New York.

• A Doctor of Physical Therapy (DPT) degree is strongly preferred.

• Certification in Basic Cardiac Life Support (BCLS) is required within 30 days of employment.

• No prior experience is necessary; however, a passion for helping others and a willingness to learn are essential.

• Prior experience or internships in home health care settings will be advantageous.

• Familiarity with patient management software and electronic health records (EHR) systems is a plus.

You can expect a competitive salary ranging from USD 100,000 to 120,000 annually, depending on experience and qualifications.
