Job description

Job Summary:

We are seeking a compassionate and motivated professional to provide Therapeutic Mentoring and Psychiatric Rehabilitation Program (PRP) services to youth and families. This role involves supporting clients in achieving their mental health and behavioral goals through structured, individualized care.


Key Responsibilities:Therapeutic Mentoring Duties:
  • Provide services to client caseload per week (PRP or therapeutic mentoring).

  • Conduct pre- and post-assessments as required.

  • Develop treatment plans tailored to client needs.

  • Document weekly sessions accurately and promptly.

  • Communicate with families as needed to support client progress.

  • Attend required staff meetings and weekly supervision.

  • Participate in trainings mandated by the Maryland Community Health Resources Commission.

  • Submit data and documentation related to grant requirements.

PRP Services:
  • Facilitate weekly one-on-one sessions (1 hour) focused on client goals outlined in the Individualized Rehabilitation Plan (IRP).

  • Write progress notes for each session.

  • Complete monthly summaries and ensure a minimum of four (4) sessions per month (onsite or offsite).

  • Lead two monthly onsite group sessions to support clients' social skills development.

  • Collaborate with clients' therapists and coordinate with community resources.

  • Update IRPs every 90 days to reflect client progress and evolving needs.

  • Attend monthly trainings (CEUs provided).
     
  • Other duties as assigned.

Qualifications
  • Education: Bachelor's Degree in human services field i.e. Psychology, Social Work, Counseling
  • Must have a minimum of (2) years of experience working with children/adolescents with mental health diagnosis.
  • Strong communication, documentation, and organizational skills.
  • Ability to work independently while maintaining professional collaboration.
