Location: US:WI:MILWAUKEE at our FROEDTERT HOSPITAL facility.

This job is HYBRID.

FTE: 1.000000

Standard Hours: 40.00

Shift: Shift 1

Shift Details: 8-430 Holidays: No Weekends: None for first 6 months on site and then can transition to 4 ten hour shifts with WFH one day a week.

Job Summary:

This Transplant Coordinator position provides primary clinical management and coordination of care for all post-transplant adult kidney, pancreas, and liver transplant patients. The Transplant Coordinator interacts with all necessary disciplines to coordinate patient services across the continuum of care. The person in this position must have exemplary customer service skills. Other duties as assigned.

EXPERIENCE DESCRIPTION:

Minimum of 3 years of acute care or dialysis experience is required. Experience relevant to Nephrology/Transplant sub-specialties is preferred

EDUCATION DESCRIPTION:

Associate's degree in Nursing with a minimum of 10 years of relevant experience is required or Bachelor's degree in Nursing with a minimum of 3 years of relevant experience is required. Bachelor's degree in Nursing is preferred.

SPECIAL SKILLS DESCRIPTION:

Analytical skills necessary in order to prepare and implement effective nursing care and develop solutions to problems concerning patient's mental or physical well-being. Ability to concentrate and pay close attention to detail for up to eighty percent of work time while: planning and performing professional nursing care, resolving patient care problems and dealing with patients, their families, and other professionals involved in the patients care, often during stressful situations.

LICENSURE DESCRIPTION:

Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) is required. Certified Clinical Transplant Coordinator (CCTC) certification, Certified Clinical Transplant Nurse (CCTN), or Acute/Critical Care Nurse (CCRN) is preferred.
