Job description

Job Requirements

General Summary


With minimal supervision from their respective HIM Manager, coordinates the activities of the health information management areas of release of information, data integrity, EMPI, document integrity, document imaging, chart analysis, transcription, death/birth certificates and physician chart completion/suspension. Performs medical record audits as required for regulatory compliance or staff performance. Acts as back up to the Site Manager and others as needed and performs those roles when applicable to that HIM setting.ย ย ย ย 


II.ย ย ย ย ย ย ย ย ย Principal Responsibilities and Tasks

The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.


  1. Coordinates the workflow of the document imaging process to include chart retrieval, prepping, and scanning, indexing and quality review.ย ย 
  2. Performs monthly quality audits on Document Imaging staff.
  3. Reports on staff productivity and provides timely feedback.
  4. Provides feedback and direction to Clinical Managers on the processing of paper to HIM.
  5. Trains all new employees on document imaging procedures.
  1. Ensures that requests for information disclosures and the completion of birth and death certificates are produced within required time frames with exceptional attention to customer service.ย 
  2. Trains physicians on the Electronic Death Registration System and facilitates the communication with the funeral homes and/or others.
  3. Collaborates with HIM Release of Information leadership on standard workflows.

3.ย ย ย ย ย Ensures that the chart completion process is timely and efficient.ย 

a.ย ย ย ย ย Provides oversight of the chart analysis function.

b.ย ย ย ย ย Generates physician record completion information as needed.

c.ย ย ย ย ย Prepares and maintains incomplete and delinquent medical record.

d.ย ย ย ย ย Lists/letters and notifies providers according to suspension protocol.

4.ย ย ย ย ย Maintains the local voice-to-text, patient identity and data integrity systems and processes.

a.ย ย ย ย ย Adds new physicians to the Transcription system.

b.ย ย ย ย ย Corrects transcription report and provider-created clinical document integrity issues.

c.ย ย ย ย ย Provides support to providers for Dragon application.

d.ย ย ย ย ย Collaborates with EMPI team on duplicate management and patient identity issues

e.ย ย ย ย ย Insures data integrity remains high and accurately reflected within the electronic medical record.

5.ย ย ย ย ย Conducts documentation audits for Joint Commission, CMS and other regulatory agencies.ย 

a.ย ย ย ย ย Reports findings to the HIM Site Manager timely and with clear detail.

b.ย ย ย ย ย Maintains working knowledge of Federal, State, HIPAA, HITECH, CMS, Joint Commission, and other regulatory agencies requirements.


Work Experience
Education and Experience

1.ย ย ย ย ย High school graduate or equivalent required. Associates degree in Health Information Management preferred.

2.ย ย ย ย ย Minimum three years' experience coordinating activities in a Health Information Management Department. Two years' performing document imaging workflow coordination in a hospital medical record department preferred.

3.ย ย ย ย ย RHIT, RHIA certifications preferred.
