Job description

Pay Range: Pay Range:$$ - $$ $$K Sign On and $$K Relocation for qualified hire! This position functions within the Urgent Care setting providing appropriate and timely nursing care for patients of all ages, promoting quality, evidenced based, and cost-effective health care outcomes. This position works within a nursing team consisting of RN's, LPN's, and Clinical Assistants. The RN will provide full assessments, prioritize patient needs/patient flow, perform skills and coordinate care as ordered by providers. In addition, the RN is expected to use critical thinking to work within standing orders to expedite care and patient services. As necessary, the RN may work in other nursing roles, so that departmental functions are covered daily. Perform duties in a responsible, professional, and compassionate manner to provide excellent patient care. Shift Details 40 hours a week rotating days and weekends Key Essential Functions and Accountabilities of the Job Perform initial triage/assessments of patients who present for urgent care services. RN also provides telephone services including an assessment and advice of self-care &/or appropriate care options. Assessment will include history of present illness, vital sign measurements as appropriate, gathering of Government Performance Results Act data, immunization status, preventative health needs and other tasks based on initial assessment and anticipation of patient &/or provider's needs. RN may take direction from provider through written, verbal and/or standing order. Administer therapeutic measures, including medication (oral, topical, IM, SQ, Intradermal, and IV push and infusion), operation and monitoring of specialized equipment, as indicated by patient needs. Initiate IV access, maintain site, monitor patient, and discontinue IV therapy. Adding secondary IV therapies, which may require mixing of a concentrated medication solution or powder. Document assessment, observation, nursing interventions, therapeutic measures, health counseling and education provided to patients either in person or by phone. Evaluate and document patient's comprehension of education. Maintain a clean and safe working environment, including adequate supplies. Upkeep of stock and equipment. Reporting/removal of any defective or missing equipment and safety hazards immediately. Adhere to infection control policy and procedures. Maintain annual competencies including safety and infection control requirements. Perform other duties as assigned, including special projects deemed necessary to provide comprehensive health services. Facilitate the coordination of patient referrals within SEARHC or other outside facilities. Track the progress of referrals and outcomes of care, providing periodic status reports to providers on outstanding referrals. Serves as the primary link for other case managers and referral coordinators in care coordination for the patients. Proactively identifies delays and obstacles to care and works to remedy individual cases and to improve the process. Contact patients for next day follow up calls and handles each situation appropriately. Work collaboratively by building bridges and creating rapport with team members within departments and across the organization. Participate in department and clinic team and improvement efforts. Participate in continued development of the clinical team. Responsible for participating in the triage role and responding to emergency situations. May serve as vaccine coordinator or secondary vaccine coordinator for clinic. Perform other duties as assigned, including special projects deemed necessary to provide comprehensive health services. Education, Certifications, and Licenses Required Graduate of an accredited school of nursing and qualified to take the NCLEX. Current full, unrestricted Registered Nursing license in Alaska or other U.S. state required. Direct Hire must have applied for an Alaska nursing license before the start date. Agency staff must have an active Alaska license. Basic Life Support (BLS) certification preferred. High school diploma or equivalent required. Clinical Competency required within 3 months of hire and every three years High Risk Competency required within 3 months of hire and every year Experience Required 2 years of general nursing experience with a minimum of 1 year in an ambulatory or urgent care clinic Knowledge of Customer service principles and recovery Safety and infection control principles Clinic protocol for following through on patient visits, treatments, and procedures. Age-appropriate vital signs, behaviors, and clinical education Skills in The nursing process and the ability to think critically. Assessment, prioritization/anticipation of needs, and data collection The use of various medical equipment Oral and written communications including competent charting. Life Saving Procedures Multi-tasking and time management Problem Solving Ability to Maintain compliance with annual competencies. Work independently while part of team, seeking help when appropriate. Move at a quick pace, stand for extended periods of time, bend, stoop, climb and move to the floor. Perform job using theory-based nursing practice model, evidence-based practices, and shared governance in a multi-disciplinary setting. Train to use various EMR systems and accurately record patient data. Provide excellent patient care to all patients from various backgrounds, ages, etc., in the urgent care setting. Communicate and interact professionally with all members of the multidisciplinary healthcare team. Remain calm, professional, and focused during stressful/urgent/emergent situations. Physical Demands: While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to stand; walk; sit; use hands to finger, handle or feel and reach with hands and arms. The employee is occasionally required to climb or balance; stoop, kneel, crouch or crawl.
