Job Description:

  • Shift - 12p-8p
  • Discusses facility core measures during handoff communication.
  • Develops individualized plans of care in collaboration with the patient, family, and healthcare providers, ensuring continuity and documentation.
  • Collaborates with interdisciplinary team members to solve admission and discharge problems.
  • Collects comprehensive patient medical history. Establishes and maintains positive relationships with patients and their families.
  • Provides family members with information about patient condition and plan of care.
  • Promotes a positive patient experience by demonstrating caring behaviors, maintaining clear communication, and collaborating with the care team to provide safe, timely, and effective whole person care.
  • Follows hospital policies and procedures, making ethical decisions based on consent and confidentiality.
  • Collaborates with nursing staff and attending physicians in establishing patient admission and discharge processes.
  • Ensures compliance with healthcare admission policies and regulations. Maintains a safe and clean environment for patients.
  • Manages all aspects of patient admission and discharge processes.
  • Prepares, completes, and registers patient admissions.
  • Other duties as assigned.

Knowledge, Skills, and Abilities:
