This is a remote part time position in the Georgia area but may require visits to one of our closest FreedomCare Offices in Georgia. Candidate MUST reside in the state of Georgia.
Overview
The Case Manager, RN is responsible for conducting initial in-home assessments, orientations for new patients and caregivers, and providing ongoing care management and support. This role combines clinical assessment, care coordination, patient and caregiver education, and quality assurance to ensure safe, effective, and person-centered care.
The Case Manager, RN collaborates closely with patients, caregivers, and interdisciplinary care teams to assess needs, develop individualized plans of care, and facilitate the seamless initiation and delivery of services. Through regularly scheduled home visits, the RN evaluates patient and caregiver well-being, monitors the quality and effectiveness of care, and identifies opportunities to enhance health outcomes, safety, and regulatory compliance. The Case Manager, RN proactively communicates clinical concerns, changes in condition, and potential barriers to care with the appropriate team members to ensure timely intervention, effective care coordination, and optimal patient outcomes.
Responsibilities
  • Conducts initial in-person clinical assessments and coordinates service activation, ensuring that all required forms and standard procedures are followed.
  • Develops, implements, manages patient's plan of care
  • making revisions and documenting changes as needed.
  • Completes all client documentation, to include written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner.
  • Actively coordinates the care of the client through communication and written documentation to the physician, and other disciplines involved in the client's care and community resources.
  • Provide ongoing coaching to drive effective utilization of established best practices, encourage caregiver education, and connect families to appropriate resources.
  • Reviews the caregiver notes daily and appropriately addresses changes in the consumer's condition or environment.
  • Ensure continuous compliance with all regulatory standards as well as company policies and procedures.
  • Performs other job-related duties as may be required or directed by the supervisor.

Education and Experience
  • Diploma from an accreditted nursing program. New hires welcome!

Is a Plus:
  • Home care case management
  • Care coordination experience

Qualifications
  • Valid and Active Registered Nursing License issued by the Georgia Board of Nursing
  • Ability to articulate cases through effective summaries.
  • Ability and flexibility to travel (usually by car) within assigned geographic area
  • valid driver's license and car insurance required.
  • Proficient in common software and web-based applications (e.g. Case Management Systems, MS Office) with the ability to teach others.
  • A passion for providing a high level of customer service, including the ability to show empathy, active listening and patience.

The base pay range for this per diem role is between $40.00 and $42.00 per hour at the commencement of your first year of employment.
