Key Responsibilities

  • Facilitate family involvement by completing discharge sessions with patients to ensure understanding of coping skills, transition plans, and available resources.
  • Confirm discharge appointments with students and their families prior to discharging from services, emphasizing the importance of follow-up care.
  • Establish and maintain communication with referral sources, outpatient providers, and school personnel to ensure continuity of care from admission and throughout treatment.
Patient Population / Client Focus

This role primarily targets school-aged children and adolescents facing various mental health challenges, including anxiety, depression, and behavioral issues. As an LCSW, you will work collaboratively with educators and parents to identify and address these challenges proactively, ensuring that each child receives the support they need to thrive both academically and personally.

Qualifications
