PRN Hospice Social Worker (LMSW / LBSW) - Sherman Area.

Concho Hearts Hospice is seeking a compassionate Licensed Medical Social Worker (LMSW or LBSW) to provide hospice social services in the Sherman area.
Our mission is to provide compassionate care wherever you call home. Our team of Nurses, Aides, Chaplains, Social Workers, Volunteers, and Doctors supports patients and families through end-of-life with dignity, comfort, and respect.

About the Role:
Our PRN Social Workers play a vital role in supporting patients and families as they navigate the emotional, social, and practical challenges of end-of-life care. You will provide counseling, resource coordination, and guidance to help patients and caregivers cope while maintaining dignity and quality of life.
You’ll work alongside a compassionate interdisciplinary team that truly values the expertise social workers bring to hospice care.
Responsibilities:
  • Assess the psychosocial needs of patients and families related to terminal illness.
  • Communicate findings with the RN Case Manager and interdisciplinary team.
  • Evaluate the patient’s home environment, financial resources, and community support.
  • Assist with funeral planning and community resource coordination when needed.
  • Develop and implement individualized social work care plans.
  • Identify bereavement needs and collaborate with the Bereavement Coordinator.
  • Participate in interdisciplinary team meetings.
  • Provide counseling and emotional support to patients and caregivers.
  • Help families navigate stress, grief, and coping challenges.
Qualifications:
  • Master of Social Work (MSW) or Bachelor of Social Work (BSW) from an accredited program.
  • Current Texas LMSW or LBSW license in good standing.
  • Minimum one year of social work experience in healthcare, long-term care, or acute care.
  • Hospice experience preferred but not required.
  • Ability to work flexible PRN hours and travel locally.
  • Comfortable using EMR and basic computer programs.
  • Ability to provide support to patients and families experiencing grief and loss.

Requirements:
  • Reliable transportation.
  • Valid driver’s license.
  • Current auto liability insurance.
  • Ability to travel between patient homes, facilities, and offices.
