Temporary Social Worker (MSW)

Compensation: $58,000 - $62,000 / year
Description
Schedule: Temporary, Full Time | Monday - Friday | 8:00 am - 4:30 pm

What You'll Do In This Role:
The Social Worker plays a vital role in empowering older adults to live safely and independently in the community. As a key member of the Interdisciplinary Team, this position provides compassionate, participant-centered case management, counseling, advocacy, and resource coordination that enhances quality of life and supports each participant's unique goals. Working with a high level of autonomy and clinical expertise under the guidance of the Social Work Manager, the Social Worker builds meaningful relationships with participants and families while connecting them to the services and support they need to thrive.
Key Responsibilities
  • Participates in the Interdisciplinary Team's initial assessments, care planning, consultation, and periodic re-assessments (minimally every 6 months) of participant's case. Assessments are to be completed prior to the scheduled team meeting. Communicates participant changes to team members.
  • Maintains current written case management records and documentation, including periodic reassessments of program participants.
  • Coordinates 24-hour care delivery. Provides individual and family counseling, develops and leads group counseling and activities.
  • Refers participants and families to appropriate community agencies or facilities, acts as liaison with such organizations and as advocate for participants.
  • Collaborate with participants to complete Advance Directives/MI post and provide assistance in obtaining and verifying current guardianship documents.
  • Complete weekly level of care and service determination audits and participates in QI processes.
  • Provide training for PCO (PACE's EMR system).
  • Obtain signature for Freedom of Choice and Annual Release documents.
  • Facilitates enrollments, disenrollment, and Medicaid applications.

Skills, Knowledge & Expertise
  • LMSW or LLMSW License in the State of Michigan.
  • 2 years' experience in a health-related area with one year working with the frail and elderly population
  • Must have reliable transportation and a valid driver's license.
  • CPR certification.
  • Position requires the ability to lift and move objects weighting up to 35 pounds without assistance.
