The department of Medicine has an exciting opportunity for a full-time The Nurse Navigator 1 who will provide clinical and operational nursing support to the Chief of Gastroenterology and the division's clinical practice. This role will assist with patient care coordination, clinical triage, procedure preparation, follow up needs, and communication between patients, providers, scheduling teams, and multidisciplinary partners.


CORE JOB FUNCTIONS

  • Triages new patient appointments to the appropriate provider(s) and assures timely scheduling of initial appointments.
  • Reviews outside medical records for appropriate scheduling.
  • Assess barriers to care and refers to support services, local, and national organizations when needed.
  • Educates on the treatment plan for patients based on diagnosis.
  • Supports patients throughout the care continuum.
  • Counsels individuals and patients on positive health practices.
  • Collaborates with a multidisciplinary team of experts to outline the best treatment for patients.
  • Performs holistic evaluation of specialty population, making use of enhanced proven techniques and procedures to achieve better results.
  • Implements the improvement of patient care, and healthcare policies and resources.
  • Mentors other healthcare professionals by functioning as a preceptor or coordinating preceptors for visiting professionals, students, new graduates, and orienteers.
  • Maintains professional knowledge by affiliating with professional and technical organizations, and participating in applicable continuing education programs, conferences, seminars, and workshops.
  • Adheres to University and unit-level policies and procedures and safeguards University assets.

Department Specific Functions

  • Supports the Chief of Gastroenterology by coordinating clinical needs, patient inquiries, and follow up related to clinic visits, procedures, testing, and treatment plans.
  • Performs clinical review and nursing triage for GI patients, including assessment of symptoms, urgency of care needs, and appropriate escalation to the provider.
  • Communicates with patients regarding plan of care, procedure preparation instructions, medication guidance, test results, follow up recommendations, and next steps as directed by the provider.
  • Assists with coordination of GI procedures, diagnostic testing, imaging, referrals, prior clinical records, and other items needed to support timely and complete patient care.
  • Collaborates with scheduling, authorization, call center, clinic, procedural, and administrative teams to help address access barriers and ensure patients are routed appropriately. Monitors and manages UChart In Basket messages, patient calls, MyChart messages, clinical tasks, and other provider related work queues in a timely and professional manner. Identifies urgent or complex clinical issues and escalates appropriately to the Chief of Gastroenterology or designated provider. Supports transitions of care by assisting with follow up after hospital discharge, emergency department visits, procedures, or abnormal findings requiring additional coordination.
  • Other duties as assigned.

CORE QUALIFICATIONS

  • Bachelor's degree in relevant field required
  • Valid State of Florida RN license required
  • Minimum 2 years of relevant experience required

  • Ability to learn new procedures, technologies, and protocols, and adapt to changing priorities and work demands.
  • Ability to work collaboratively with others and contribute to a team environment.
  • Skilled in using office software, technology, and relevant computer applications.
  • Strong and clear written and verbal communication skills for interacting with colleagues and stakeholders.
