Job description

Community Living Supervisor / Quality Coordinator

Duties and Responsibilities  

1. Leadership and Supervision 

a. Coach and evaluate Community Living Supervisors to ensure consistent and effective leadership across community living sites. 
b. Assist in the recruitment, hiring, and onboarding of new staff. 
c. Monitor staffing patterns across multiple sites to ensure adequate coverage and effective scheduling practices. 
d. Promote a culture of accountability, teamwork, and professional development. 

   2. Program Oversight and Service Quality 

a. Ensure the delivery of person-centered services that uphold client rights, choices, and individualized planning. 
b. Oversee service planning processes, ensuring quality implementation of Individual Service Plans (ISPs). 
c. Participate in interdisciplinary meetings, enrollment processes, along with quarterly and annual client ISP. 
d. Address escalated client or staff concerns in collaboration with supervisors and other departments. 

   3. Quality Assurance & Compliance 

a. Develop and implement systems for internal audits and quality assurance, including review of documentation, medication records, safety protocols, and financial records. 
b. Monitor compliance with agency, state, federal, and accrediting body regulations across all assigned sites. 
c. Analyze audit findings and implement corrective action plans where needed. 
d. Track program metrics and provide reports to leadership regarding trends, risks, and successes. 
e. Participate in incident investigations and ensure timely reporting and follow-up. 

   4. Training and Development 

a. Identify training needs and develop strategies for ongoing professional development of supervisors and direct care staff. 
b. Lead or coordinate initial and ongoing training initiatives, including Hope Haven University, new supervisor onboarding, and policy refreshers. 
c. Mentor Community Living Supervisors in leadership, decision-making, performance management, and person-centered planning. 
d. Promote a learning culture through feedback, coaching, and recognition. 

   5. General Responsibilities 

a. Uphold and promote the mission and values of Hope Haven across all program areas. 
b. Foster collaboration and effective communication with all departments, including Human Resources, Business Office, and Nursing. 
c. Serve as on-call backup for Community Living Supervisors and respond to urgent or emergency situations. 
d. Participate in agency-wide initiatives, leadership meetings, and strategic planning sessions. 
e. Ability to travel between services sites regularly. 

f. Other Duties as assigned.

 

Requirements:

  • High School graduate, (BA or BS degree in Human Services field is preferred
  • 5 years of experience in the human services field, including 2 years in a supervisory role.
  • Ability to make sound decisions in all staff and programming matters
  • Take and pass the required medication administration course
  • Valid Class C driver’s license required, unless driving a vehicle with a capacity of 8 or more, then a Class “D” license will be required and be an insurable driver with our insurance company
  • Clearance from all applicable background checks
