Nurse Practitioner (NP) or Physician Assistant (PA)

As a Nurse Practitioner (NP) or Physician Assistant (PA) at Centerwell Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care. We're not just improving healthcare—we're improving lives. That includes yours. You will work on site at our Centerwell Poinciana. You will report to our associate medical director.

Required Qualifications

  • Master's Degree as a nurse practitioner or completion of a physician assistant program with board certification.
  • Active, unrestricted NP or PA license in the state of Florida
  • Two years of experience in value-based care, geriatric or primary care, managing adults aged 65+ with chronic conditions
  • Proficiency using an EMR
  • Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $123,800 - $170,300 per year. This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
