Founded in 1968, Kualoa-Heʻeia Ecumenical Youth (KEY) Project is a nonprofit community organization dedicated to nurturing the well-being of people and ʻāina by supporting, serving, and sustaining our community. Serving residents from Waimea to Waimānalo, KEY Project provides programs that strengthen food security, cultural connection, youth development, kūpuna wellness, and disaster resilience. As a trusted community resource and member of the Koʻolau Resilience Hub Network, KEY Project works alongside residents before, during, and after emergencies to help individuals, families, and communities recover and thrive.
Position Summary
The Disaster Case Manager provides compassionate, culturally responsive case management services to individuals and families affected by the Kona Low Storm emergency and others as assigned. The Disaster Case Manager helps disaster survivors navigate recovery by connecting them with resources, advocating on their behalf, and developing individualized recovery plans that support long-term stability and resilience.
Essential Duties and Responsibilities
  • Conduct outreach to identify and engage individuals and families impacted by disasters, including vulnerable and underserved populations.
  • Complete comprehensive intake assessments using a strengths-based, trauma-informed approach.
  • Develop individualized Disaster Recovery Plans that identify recovery goals, available resources, and action steps.
  • Provide ongoing case management services from intake through case closure.
  • Maintain an active caseload of approximately 30-35 households, depending on case complexity.
  • Conduct regular follow-up with clients through home visits, telephone calls, and community meetings.
  • Monitor client progress and modify recovery plans as needed to support successful outcomes.
  • Respond promptly to client questions and requests for assistance.
  • Connect clients with local, state, federal, and nonprofit resources, including housing assistance, financial support, healthcare, behavioral health, food assistance, and disaster recovery programs.
  • Assist clients with completing applications for FEMA, insurance claims, public benefits, and other recovery resources.
  • Advocate for equitable access to services, particularly for kūpuna, Native Hawaiian families, individuals with disabilities, and households facing significant barriers.
  • Coordinate services with community organizations, government agencies, and recovery partners to prevent duplication of benefits and maximize available assistance.
  • Maintain timely, accurate, and confidential case records using the designated case management system.
  • Document all client interactions within required program timelines.
  • Track client outcomes and progress toward recovery goals.
  • Prepare monthly reports, data summaries, and other required program documentation.
  • Maintain confidentiality of all client information in accordance with applicable federal, state, and organizational policies.
  • Ensure compliance with all applicable federal, state, county, and grant requirements, including FEMA and disaster recovery program guidelines.
  • Participate in program monitoring, audits, training, and quality assurance activities.
  • Maintain accurate records supporting program and financial accountability.
  • Perform other duties as assigned to support disaster response, recovery operations, and KEY Project.

Qualifications
Required
  • Bachelor's degree in Social Work, Human Services, Public Health, Psychology, Emergency Management, or a related field; or equivalent combination of education and experience.
  • Minimum one (1) year of experience in case management, social services, disaster recovery, community outreach, or related human services.
  • Knowledge of trauma-informed care, strengths-based case management, and community resource navigation.
  • Strong organizational, communication, and documentation skills.
  • Proficiency with Google Suite and electronic case management systems.
  • Ability to work independently and collaboratively in fast-paced and evolving environments.
  • Knowledge of Native Hawaiian and other Polynesian culture, values, and the diverse communities of Koʻolau.
  • Valid Hawaiʻi driver's license, reliable transportation, and current automobile insurance.

Preferred
  • Experience with FEMA Individual Assistance, Disaster Case Management, or emergency response programs.
  • Familiarity with VisionLink or comparable case management software.
  • Experience working within nonprofit organizations or community-based human services.
  • Bilingual or multilingual skills are highly desirable.

Physical Requirements
  • Ability to travel regularly throughout the service area.
  • Ability to conduct home visits and attend community meetings.
  • Occasionally lift and carry materials weighing up to 25 pounds.
  • Ability to work evenings, weekends, or extended hours during disaster response and recovery operations.

Compensation
Starting Pay: $25-$30 per hour
Employment is contingent upon grant funding.
Background Screening
Employment is contingent upon successful completion of a criminal background check. Individuals must have no record of offenses involving children, elders, or other vulnerable populations.
