Job description

JOB PURPOSE:
The Disaster Case Manager is responsible for supporting individuals and families affected by disasters in their recovery journey. This role involves conducting client outreach, assessing needs, developing recovery plans, and connecting clients to appropriate resources and services. The Disaster Case Manager ensures clients' needs are met in a timely, efficient, and empathetic manner while maintaining accurate documentation and adhering to program requirements.
ESSENTIAL JOB RESPONSIBILITIES:
โ€ข Conduct outreach to disaster-affected clients.
โ€ข Complete eligibility assessments and the intake process.
โ€ข Conduct home and community visits to provide ongoing support.
โ€ข Collaborate with clients to develop comprehensive recovery plans addressing unmet disaster-caused needs.
โ€ข Monitor progress and assess the effectiveness of services through follow-up visits.
โ€ข Assist clients in identifying and securing available benefits, community resources, and social services.
โ€ข Act as a liaison among family services, health professionals, and other stakeholders to address client needs effectively.
โ€ข Maintain accurate and detailed records of client background, case history, and progress towards recovery goals.
โ€ข Utilize online software to document and track case information.
โ€ข Prepare reports as requested by the Case Management Supervisor or Program Manager.
โ€ข Meet regularly with the Case Management Supervisor to review caseload and receive guidance.
โ€ข Provide ongoing program evaluations and suggest improvements to enhance service delivery.
โ€ข Participate in workshops, seminars, and other educational activities to foster professional growth.
โ€ข Provide status updates and reports on assigned cases as needed.
โ€ข Perform additional duties as assigned to support the mission and goals of the program.

ESSENTIAL QUALIFICATIONS:
EDUCATION:
โ€ข Preferred: Bachelor's degree in behavioral sciences, human services, or social services.
โ€ข Required (in lieu of degree): High School diploma or GED with three (3) years of relevant experience.
EXPERIENCE:
โ€ข Bachelor's degree holders: Minimum of two (2) years of experience in child welfare, emergency disaster response, and/or recovery environments. Experience working in multi-disciplinary teams to develop case plans is strongly preferred.
โ€ข High School diploma/GED holders: Relevant experience in child welfare, emergency disaster response, and/or recovery environments is required.
โ€ข Additional Skills: Bilingual communication skills (English/Spanish) are preferred.
ATTENDANCE: Must maintain regular and reliable attendance as determined by the employer's standards.
LICENSES: Driver's License with clear record required.
OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must be available and willing to work nights, weekends and holidays as required to meet business needs. Including deployment within 24 hours to disaster locations for as long as 2-4 weeks.
