HOSPICE LICENSED PRACTICAL NURSE (LPN)
A Path of Care Hospice & Home Health in Weatherford, OK is seeking a PRN Hospice Licensed Practical Nurse (LPN) to provide compassionate, skilled care to patients in their homes. What You'll Do as a Hospice LPN
As a Hospice LPN, you provide direct patient care under the supervision of the RN Case Manager and play a vital role in supporting patients and families. Your responsibilities include:
  • Delivering care according to the individualized plan of care
  • Assessing patient needs and reporting changes in condition
  • Identifying and reporting any signs of abuse
  • Making recommendations for updates to the plan of care
  • Counseling patients and families on advance directives such as living wills and healthcare surrogate forms
  • Maintaining accurate, up-to-date patient records
  • Building positive relationships with vendors and community partners
  • Participating in Total Quality Improvement initiatives and training programs
  • Assisting with new nurse orientation and discharge planning
  • Participating in the on-call rotation as needed

Your compassion and clinical skill help patients and families navigate end-of-life care with dignity and support.
Qualifications
  • Active Oklahoma LPN license
  • At least 1 year of clinical experience
  • Ability to care for patients and families facing terminal illness
  • Valid Oklahoma driver's license, reliable transportation, and vehicle insurance
  • Acute care experience preferred but not required
  • Strong interpersonal and communication skills
  • Ability to work effectively as part of a team

Work Schedule
  • Flexible hours between Monday and Friday, 8:00 AM - 5:00 PM
