CHC is looking for a dedicated Registered Nurse to join our Family Practice team at our brand-new Muscatine clinic! In this position, you will learn a variety of skills while providing comprehensive, patient-centered care to individuals and families in our community.
Salary: $31-$40/hour
In this key role, you will:
  • Health Coach - Develop care plans and partner with patients to achieve their health goals
  • Patient Advocate - Serve as a liaison between patients, families, providers, hospitals, and community agencies
  • Connector - Identify and link patients to resources that support access to care
  • Social Determinants of Health - Recognize and address psychosocial, cultural, ethnic, and spiritual needs of patients and families
