Job description

Position Requirements

-Must have valid LPN certification (completed an approved LPN training program and pass the National Council Licensure Examination for Practical Nurses) (must be registered with IDFPR)

-All educational and licensing credentials must be available and on file to meet the DCFS requirements to review the cases of children.

-Experience with school-age children is preferred, requiring the ability to communicate professionally both verbally and in writing.

-Meets all applicable employment regulations set by the Illinois Department of Children and Family Services and other regulatory agencies.

-Must have the ability to respond appropriately to the cultural differences present among the agency's service population.

General Job Description

The Child Link LPN will review the agency files for the youth in care to create a Health Report Summary outlining the youth’s health-related history, current conditions, treatments, and, as needed, instructions for the foster parents.

ESSENTIAL DUTIES:

- Around 25 files will be reviewed weekly, and a summary will be prepared titled: Health Report Summary for each youth file reviewed. These will outline the youth’s treatment goals, current health status, medical treatment, therapeutic recommendations, and foster parent instructions for treatment or next steps.

​- The Health Reports will be submitted to the Supervisor and addressed by the Case

​Manager and the Supervisor.

​​​- Ensure Medical Logs are filled out correctly following prescribed medication

​​​-Participation in discussions at staff meetings regarding treatment and recovery goals

​​​and objectives for youth assigned.

​​

​​-Responsible for utilizing opportunities within the shift to arrange for needed mental

​​Health, medical, hearing and dental appointments needed for the youth in the

​​Programs.

DOCUMENTATION:

-completes all required documentation, such as: Health Report Summaries, appointment confirmation sheets and daily sheets in a professional manner, i.e., complete, accurate, legible, and submitted in a timely manner at the end of each day.

STAFF DEVELOPMENT:

-participates in required trainings, including CPR and First Aid.

-attends team meetings and trainings as scheduled

-meets with Supervisor as scheduled; comes to supervision prepared with questions, suggestions, and concerns

-must renew and maintain a valid RN license for the State of Illinois

OTHER DUTIES as assigned by the immediate supervisor or administrative staff
