Job description


We are seeking a compassionate and adaptable Registered Nurse to join our Street Outreach Team. This role is part of the HRSA Street Medicine Intervention for people with HIV who are unsheltered, and is designed to bring critical health services directly to people experiencing homelessness and at-risk for HIV and Hepatitis C.
The RN Street Outreach Nurse will provide brief health assessments, first aid, and vital signs monitoring in non-traditional settings, while also connecting individuals to ongoing care and resources. This position requires flexibility, empathy, and the ability to work collaboratively across agencies and with vulnerable populations.
Position Overview Grant-Funded Temporary Position Disclaimer:
This is a temporary position funded entirely through the HRSA Street Medicine Intervention Program. The grant is scheduled to expire on July 31, 2029, and this position will end on that date. Continued employment is contingent upon the ongoing availability of grant funds. If funding is reduced or discontinued, this position may be subject to reduced hours, reassignment, or termination.
Key Responsibilities:
  • Provide basic health assessments during street outreach, including first aid and vital signs monitoring
  • Assist in connecting individuals to HIV, Hepatitis C, and other health services
  • Collaborate with an interagency outreach team to deliver coordinated, person-centered care

Schedule:
  • Part Time
  • Primarily Day Shift
  • 20 Hours Per Week
  • Occasional weekend and evening hours required

Location & Travel:
  • Position works closely with the team at Connection Café in Connersville
  • Regular travel throughout the East Region, with a primary focus on Fayette County

Qualifications:
  • Requires an Associates of Nursing (ASN); Bachelors of Nursing (BSN) preferred.
  • Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
  • Requires that the RN has graduated from a nationally accredited nursing program. Exceptions: Candidates who have not graduated from a nationally accredited program may be considered if they possess more than two years of acute care experience. Such exceptions are subject to review and approval by the Chief Nursing Officer (CNO).
  • Requires Basic Life Support certification through the AHA.
  • Other advanced Life Support certifications may be required per unit/department specialty according to patient care policies.
