Your Opportunity:

Integrated Home Care uses a strengths-based Client Centered Case Management practice framework. Using a person-centered approach you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals. In collaboration with the client/family and interdisciplinary team you will ensure services are provided and modified on an ongoing basis, based on client's response to interventions, incorporating stewardship into service authorization. You will report to the Home Care Manager and will be responsible for coordinating and providing quality care to clients in the community. You will provide assessments, case coordination and direct care in partnership with clients and their families in their own residence or an alternate care setting, as determined by the assessment. The successful incumbent will be expected to demonstrate proficiency in the following areas: personal and professional accountability, systems thinking, creative thinking, communicating and building relationships, organizational and time management skills, interpersonal skills and intermediate level of computer skills.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

The candidate must have at least 2 years of Social Worker experience with one year in a community based setting (Home Care experience preferred). Must have current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) from Heart and Stroke Foundation within the last 12 months. This role requires travel; possession of a valid drivers' license and access to a motor vehicle in good repair for work related purposes is a requirement of this position. You will be required to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. For example: lifting of approximately 5-10 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common within this job. Learning resources and training are provided to safely perform work.

Preferred Qualifications:

The ideal candidate will have experience with mental health, addictions, trauma informed care and complex client behaviors. Additional experience working with acute clients and/or long term clients with a focus on chronic disease management, recent experience in a community based setting. Home Care experience is an asset.
