Job Description

We are seeking a driven, professional Healthcare Referral Liaison to build relationships with hospital discharge planners, social workers, case managers, rehabilitation centers, and nursing facilities throughout Collier County.

This role is responsible for generating qualified referrals and scheduling in-home assessments with our Assessment Coordinator.

You will be the face of our 24/7 emergency home care response team.
