Job Responsibilities for the Registered Nurse Include:
  • Provide patient care through assessment, triage, planning, intervention, and evaluation of patient responses.
  • Ensure timely communication with providers and care teams to coordinate care for high-risk patients.
  • Deliver transitional care management to prevent unnecessary hospital readmissions, including medication reconciliation.
  • Communicate with patients, families, PCPs, and care teams post-hospitalization for follow-up care.
  • Review patient needs prior to office visits to maximize effectiveness (chronic disease protocols, preventive health, counseling).
  • Coordinate medication reviews with providers, ensuring adherence and patient understanding.
  • Review lab results daily and discuss abnormalities with ordering providers.
  • Assist patients in setting and maintaining self-management goals, addressing both physical and behavioral health.
  • Recommend interventions to modify health-risk behaviors and collaborate on meaningful treatment goals.
  • Provide nursing care for patients with diverse conditions, illnesses, or injuries.
  • Maintain compliance with documentation, coding, and productivity requirements.
  • Educate patients on disease prevention, screenings, and health promotion across the lifespan.
  • Participate in multidisciplinary care teams, performance improvement activities, and continuing education.
  • Ensure compliance with policies, procedures, and regulatory standards (FQHC, NCQA, HIPAA).
  • Maintain licensure, certifications, and continuing education requirements.
  • Uphold patient rights and deliver care in a safe, clean environment.
Requirements of the Registered Nurse:
  • Associate Degree in Nursing (ADN) required; Bachelor's Degree in Nursing (BSN) preferred.
  • Valid State of Michigan RN licensure required.
  • BLS certification required; ACLS certification preferred.
  • Minimum of two (2) years of recent nursing experience in a community-based setting (FQHC or Integrated Care) preferred.
  • Experience working with patients with severe and persistent mental illness; comfortable with community outreach.
  • Must meet all Continuing Education requirements.
  • Must be able to work cooperatively with staff and others involved in the delivery of services to patients.
  • Experience in managing multiple tasks simultaneously and meeting critical deadlines.
  • Ability to work cooperatively with staff and maintain confidentiality per HIPAA guidelines.
  • Knowledge of PCMH preferred.
  • For intake and case management services: Qualified Mental Health Professional (QMHP) - Individual with specialized training3 (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating or working with a person who has mental illness; AND is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, behavior analyst, registered nurse, therapeutic recreation specialist, licensed/limited-licensed professional counselor, licensed or limited licensed marriage and family therapist, a licensed physician's assistant, OR a human services professional with at least a bachelor's degree in a human services field.
  • Health Care Professional - Health Care Professional - A physician, registered nurse, physician assistant, nurse practitioner, clinical nurse specialist, or dietician. Services provided must be relevant to the health care professional's scope of practice.
Skills:
  • Must demonstrate an ability to remain calm and perform effectively and professionally during peak periods of activity and in emergency situations
  • Proficiency with Electronic Medical Records (EMR) and ICD-10 coding.
  • Strong critical thinking, problem-solving, and communication skills.
  • Ability to initiate and maintain positive working relationships with CCH staff and other organizations
PHYSICAL DEMANDS/WORKING CONDITIONS:
  • Must be able to lift 20 pounds and work flexible hours, including evenings and weekends. Hours as determined by the organization and our patient population. Hours are subject to change based on need.
  • Work hours may shift dependent on project or program needs and staff availability. May be reassigned to other areas during periods of low census days or where the need is greater for the day.
  • Must maintain acceptable background check clearance, certifications, and licensure.
  • Walking, sitting, and/or standing for prolong periods of time may be required.
  • Normal office and medical office environments with exposure to noise, dust, or extreme temperatures.
  • Interacts with a patient population exhibiting severe and persistent mental illness.
  • May have close contact with clients and/or their living environments which could include potential exposure to contagious diseases, unpleasant odors and/or to bug/pest infestations.
  • Must possess and maintain an acceptable background check clearance, certifications, licensures and/or training as required by Central City Health, federal/state/county agencies, and other entities.
