At Nascentia Health, youll provide skilled, compassionate care to adults with chronic, acute, or complex medical needs in the comfort of their homes, helping patients maintain independence, improve quality of life, and feel supported in their own environment. We believe in listening to our nurses, supporting them, and setting them up for success every day. Home visits are scheduled MondayFriday, 8:00 AM4:30 PM, with no on-call requirement. This Full-Time position offers flexible scheduling and a full benefits package!

What Youll Do as a Resident!

You will learn how to provide high-quality nursing care to adult patients through a combination of hands-on clinical care, assessment, education, and care coordination. Youll be trusted, valued, and empowered to use your clinical judgment to make a meaningful difference.

Your responsibilities include:

  • Deliver safe, patient-centered nursing care in the home
  • Serve as the primary case manager, coordinating care with the interdisciplinary team
  • Develop and implement individualized plans of care
  • Provide patient and caregiver education, support, and disease management teaching
  • Ensure accurate, timely documentation (documentation time built into the home visit)
  • Supervise and provide oversight of paraprofessional staff, including home health aides, ensuring tasks are performed safely and according to the patients plan of care
  • Maintain compliance with all infection control and regulatory standards
  • Stay current with clinical best practices

What Were Looking For!

  • Valid drivers license, auto insurance, and reliable vehicle
  • Self-directed, organized, and comfortable working independently
  • Strong communication and computer skills
  • Current New York State registered professional nurse license or may hold a Graduate Nurse License; BSN preferred (tuition reimbursement available!)
  • Must attain RN licensure within 90 days of hire.
  • Self-directed and organized with the ability to work independently.
  • Basic computer knowledge with proficiency in Microsoft Office or other Word processing, spreadsheet, and database management.
  • Good oral and written communication skills.
  • Demonstrates motivation to learn a new role and is an active learner.

Physical Requirements:

  • Ability to sit or stand for long periods during work day
  • Ability to perform tasks involving physical activity (including but not limited to): Pushing/pulling on average 50 lbs. with a max of 150 lbs.; lifting/carrying on average 25 lbs. with a max of 50 lbs.; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs.
  • Ability to drive in ALL weather conditions (rain, sleet, snow, wind).
  • Visual/hearing ability sufficient to comprehend written or verbal communication
  • Ability to use telephone and communicate over the phone
  • Frequently uses a laptop computer to enter orders and obtain product information
