At Nascentia Health, you'll provide skilled, compassionate care to children with complex medical needs in the comfort of their homes, helping families feel supported and helping young patients thrive. We're a team that believes in listening to our nurses, supporting them, and setting them up for success every single day. Home visits are scheduled Monday-Friday, 8:00 AM-4:30 PM. There is no on-call requirement. This Full-Time position offers flexible scheduling with a full Benefit Package!

What You'll Do as a Pediatric RN Case Manager
Provide high-quality nursing care to children and support families through a mix of hands-on clinical care, assessment, education, and care coordination. You'll be trusted, valued, and empowered to use your clinical judgment to make a meaningful difference.
Your responsibilities include:
  • Deliver safe, family-centered pediatric nursing care in the home
  • Serve as the primary case manager, coordinating care with the interdisciplinary team
  • Develop and implement individualized plans of care
  • Provide family teaching, support, and education
  • Ensure accurate, timely documentation (Documentation time built into the home visit)
  • Supervise and provide oversight of paraprofessional staff, including home health aides, ensuring tasks are performed safely and according to the patient's plan of care.
  • Maintain compliance with all infection control and regulatory standards
  • Stay current with clinical best practices

What We're Looking For!
  • Current NYS Registered Nurse license (accredited program)
  • BSN preferred (tuition reimbursement available!)
  • Valid driver's license, auto insurance, and reliable vehicle
  • Self-directed, organized, and comfortable working independently
  • Pediatric or maternal/child health experience a plus
  • Strong communication and computer skills

Physical Requirements:
  • Ability to sit or stand for long periods during work day
  • Ability to perform tasks involving physical activity (including but not limited to): Pushing/pulling on average 50 lbs. with a max of 150 lbs.; lifting/carrying on average 25 lbs. with a max of 50 lbs.; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs.
  • Ability to drive in ALL weather conditions (rain, sleet, snow, wind).
  • Visual/hearing ability sufficient to comprehend written or verbal communication
  • Ability to use telephone and communicate over the phone
  • Frequently uses a laptop computer to enter orders and obtain product information
  • Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.
