The Care Manager is a central hub for care coordination, compliance, staffing and scheduling, Client communication, and Branch operations. ROLE RESPONSIBILITIES

What you'll own and lead as a: Care Manager -


Provide 5-star service to all incoming calls, serving as a key communication hub for Clients, families, Caregivers, Case Managers, and referral partners

Coordinate care services to ensure timely, consistent, and appropriate service delivery for all assigned Clients

Develop individualized service plans and conduct initial and ongoing Client assessments for quality and compliance

Prepare Client welcome materials, intake documentation, and manage authorizations to support billing accuracy

Support scheduling and staffing to minimize overtime, reduce service disruptions, and ensure consistent shift coverage

Provide after-hours support as needed to maintain seamless care continuity

Maintain Branch compliance efforts, including compliant licensing documentation

Conduct weekly New Hire Orientation sessions for incoming Caregivers


ROLE QUALIFICATIONS

Experience in home care, healthcare coordination, or a client-facing service role preferred

Highly organized with strong attention to detail and accurate record-keeping practices

Proactive communicator with excellent interpersonal skills and a warm, professional presence

Solutions-oriented and self-managed — able to handle multiple priorities in a fast-paced environment

Committed to compliance, HIPAA standards, and maintaining accurate documentation

Strong collaboration skills with a team-first mindset and high-quality customer service approach

Proficient with technology and healthcare management or scheduling systems

Experience in assisted living, in-home care, or billing and authorization management a bonus
