Job description

PSYCHIATRIC CLINIC LEAD-NURSE PRACTIONER

The Clinical Lead - Nurse Practitioner (NP) will work in collaboration with TeamCare primary care physicians to ensure proper oversight and coordination of patient care. The role’s core responsibilities entail: gathering detailed histories, performing physical exam and conducting relevant diagnostic tests, diagnosing and treating common medical conditions within their scope of practice, managing patients with chronic illnesses and tailoring treatment plans accordingly, educating patients and their families about disease prevention, health promotion, and slow progression of chronic illnesses, referring to specialists when medically warranted, documenting accurate and complete patient records in the Electronic Health Records (Athena), adherence to professional standard professional conduct, participating in quality improvement initiatives to enhance patient care and outcomes, and work effectively within a healthcare team including healthcare professional, colleagues, patients and their families.


CAQH, DEA License, and Medicare & Medicaid IDs are a must, Homecare experience preferred or willing to do homecare.


Location: Telehealth [must be available to come in person for special cases]. (Orientation in the Bronx)


Schedule: Monday - Friday 8:00am-5:00pm (1 hour lunch) (FT)
