Position Summary:

The Lead Case Manager will maintain a caseload and be responsible to receive case manager’s documentation on a weekly basis in order to approve and process services for billing and payment to rendering provider. Each Targeted Case Manager will be assigned specific clients at time of intake and be required to program and monitor such things as basic needs, academic issues, court appearances, doctor appointments, clinical sessions and discharge referrals. The Targeted Case Manager will also maintain open communication with family, courts and others as necessary. The Targeted Case Manager will meet with members of the Treatment Team as needed.

Essential Duties and Responsibilities:

1. Receive timesheet and progress notes from case managers assigned to lead case manager on a weekly basis.

2. Monitor and review the completion of all TCM assessments and paperwork such as service plans, service plan reviews, case notes at the time requested in the Medicaid manual.

3. Turn in timesheets weekly according to billing requirements. 

4. Complete Employee Performance Report on a weekly basis and submit to immediate supervisor. 

5. Provides training, consultation, and problem-solving assistance to Case Managers.

6. Consults with Case Managers on difficult cases and provides a second opinion, expertise and problem solving assistance. 

7. Perform other related duties as assigned by Case Manager Supervisor


Requirements:

Education and/

or Experience: Bachelor’s degree in Psychology, social work, mental health or related field. Be employed by Agape Network as a Targeted Case manager for at least 3 months and have a satisfactory evaluation during this period.
