**Family Nurse Practitioner Opening โ€“ Hammond, IN**
Located in Hammond, Indiana
Full-time, exempt Nurse Practitioner opportunity providing medical services to patients of all ages within scope of licensure, training, and credentialing under physician supervision.
**Position Overview**
-Full-time exempt position
-Provide care to patients of all ages
-Work under supervision of collaborating physician
-Practice within scope of licensing, specialty, and credentialing
-Must meet organizational credentialing requirements
-Must demonstrate clinical competency in area of medicine
-Team-based, collaborative care model
-Office-based clinical environment
**Essential Clinical Responsibilities**
-Obtain relevant health and medical histories
-Identify medical risks and patient needs
-Update and document changes in health status
-Order appropriate diagnostic testing
-Develop and implement treatment plans of care
-Prescribe appropriate pharmacologic therapies
-Prescribe non-pharmacologic interventions
-Develop and provide patient education plans
-Make referrals to specialists and community agencies as appropriate
-Schedule follow-up appointments per best practice standards
-Document patient outcomes to evaluate effectiveness of care plans
-Reassess and modify treatment plans as necessary to achieve goals
-Provide education to promote optimal health
