Family Systems Navigator - Mobile Response Team (MRT)
Position Summary
South County Mental Health Center, Inc. is seeking a compassionate and collaborative Family Systems Navigator to join our Mobile Response Team (MRT). This position helps families involved with the child welfare system navigate available services, coordinates care across multiple systems, and serves as a liaison between MRT and community partners to reduce gaps in care.

South County Mental Health Center, Inc. is a private, non-profit, Joint Commission-accredited behavioral healthcare provider serving Southeast Palm Beach County. We are dedicated to delivering compassionate, high-quality mental health services to individuals and families in our community.
Minimum Qualifications
  • Bachelor's degree in a human services-related field.
  • Minimum five (5) years of experience working within children's systems.
  • Strong working knowledge of Child Welfare, School Systems, and Juvenile Justice.
  • Strong communication, collaboration, and organizational skills.
  • Ability to work effectively with families, system partners, and multidisciplinary teams.

Preferred Qualifications:
  • Experience working with Mobile Response Teams or community-based behavioral health services.
  • Experience working with children, families, foster families, or youth involved in child welfare.
  • Experience coordinating services across multiple community systems.
  • Experience providing training, technical assistance, or coaching to staff.

Duties and Responsibilities
Family Navigation & Care Coordination
  • Assist parents and families involved with child welfare in navigating the child welfare system.
  • Help reduce fragmentation of care by coordinating services and connecting families with appropriate resources.
  • Serve as a navigation resource for families and system partners.
  • Work collaboratively with Child Welfare to assist foster parents in preventing placement disruptions.
System Partnerships & Collaboration
  • Develop and maintain collaborative relationships with system partners.
  • Serve as a liaison and navigation coordinator with Child Welfare, the Department of Juvenile Justice, and School Districts.
  • Collaborate with local law enforcement, hospitals, Baker Act receiving facilities, civil diversion programs, and other community systems as needed.
  • Attend system partner meetings where MRT representation is needed.
Outreach & Education
  • Conduct outreach to system partners to increase awareness of MRT services.
  • Provide education regarding the benefits and appropriate use of Mobile Response Services.
  • Support the development of Memorandums of Understanding and Collaborative Agreements with community partners.
  • Participate in community and system-level initiatives related to children's behavioral health services.
Staff Support, Training & Technical Assistance
  • Provide technical assistance to MRT staff regarding Child Welfare, School Systems, and Juvenile Justice.
  • Attend trainings and staffing meetings with system partners.
  • Share relevant information and education with MRT staff.
  • Serve as a coach and resource to MRT staff regarding system needs and navigation.
Additional Duties
  • Attend required trainings and meetings.
  • Perform other duties and responsibilities as assigned.
