• Conduct thorough evaluations of patients, including the assessment of medical history, physical abilities, and functional limitations to formulate appropriate treatment plans.
• Clearly explain evaluation findings and treatment plans to both patients and their families, ensuring that they are active participants in goal setting.
• Implement individualized treatment protocols based on assessment findings, while continuously monitoring patient progress and adapting interventions as necessary.
• Collaborate with other healthcare professionals to ensure comprehensive patient care, recognizing any changes in the patient’s condition that may necessitate modifications to their treatment plan.
• Manage discharge planning effectively and provide follow-up care recommendations, documenting all patient care activities accurately and in compliance with healthcare regulations.
• Maintain effective time management and productivity standards, ensuring punctuality for meetings and appointments.
• Contribute to departmental goals and initiatives, actively participating in community outreach and marketing activities to promote our services.
• Engage in continuous professional development activities, pursuing additional training, certifications, and serving as a resource for student clinical education.

At Swift Health Physical Therapy, our patient population comprises individuals from various backgrounds and age groups requiring rehabilitation for musculoskeletal injuries, post-surgical recovery, neurological conditions, and chronic pain. • Completion of a Doctorate degree in Physical Therapy (DPT) from an accredited program.
• Current and valid state license to practice as a Physical Therapist in New Jersey.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• Strong communication skills and a passion for delivering high-quality patient care.

• While a minimum of 2 years of experience is preferred, we welcome applications from new graduates eager to advance their careers.
