Job description

Crisis Stabilization Unit (CSU) Social Worker
Position Summary
South County Mental Health Center, Inc. is seeking a compassionate and skilled Social Worker to join our Crisis Stabilization Unit (CSU). This position is responsible for providing psychosocial assessments, discharge planning, care coordination, and community linkage services for individuals experiencing mental health crises. The Social Worker works closely with clients, families, and the multidisciplinary treatment team to ensure successful stabilization and transition to ongoing care.

Minimum Qualifications
  • Bachelor's Degree in Psychology, Social Work, or a related field from an accredited college or university
  • Minimum one (1) year of social work experience working with chronically mentally ill, dually diagnosed, or emotionally disturbed adults
  • Valid Florida Driver License
  • CPR Certification
  • Strong clinical assessment, case management, and discharge planning skills
  • Working knowledge of behavioral health documentation and treatment planning

Preferred Qualifications:
  • Master's Degree in Social Work (MSW), LMSW, or LCSW
  • Two (2) or more years of social work experience in a behavioral health setting
  • Experience working in crisis stabilization, inpatient, or acute care environments
  • Knowledge of community resources, housing services, and outpatient treatment programs

Duties and Responsibilities
Client Assessment & Care Coordination
  • Complete intake and psychosocial assessments within required timeframes
  • Assess client and family needs and identify appropriate services and supports
  • Develop individualized treatment plans within 1-3 days of admission
  • Participate in daily multidisciplinary treatment team meetings
  • Attend weekly social work meetings and care coordination activities
Discharge Planning & Community Linkage
  • Develop comprehensive discharge plans and recommendations
  • Coordinate referrals for housing, outpatient treatment, and community services
  • Link clients to appropriate resources and support systems following discharge
  • Coordinate discharge planning with nursing, medical providers, and community partners
  • Transport clients when necessary to support discharge needs
Documentation & Record Keeping
  • Maintain accurate and timely client records
  • Document assessments, treatment planning activities, and client contacts in the medical record
  • Complete discharge goals and required clinical documentation
  • Ensure compliance with agency, regulatory, and documentation standards
Training & Professional Development
  • Attend staff development, in-service trainings, and departmental meetings
  • Assist with training and mentoring practicum students as assigned
  • Participate in ongoing professional development opportunities
Additional Duties
  • Perform other duties and special projects as assigned by leadership

Core Competencies
  • Strong clinical assessment and counseling skills
  • Knowledge of severe and persistent mental illness and co-occurring disorders
  • Understanding of Baker Act, Marchman Act, client rights, confidentiality, and abuse reporting requirements
  • Advanced case management and discharge planning abilities
  • Excellent written and verbal communication skills
  • Strong organizational and time management skills
  • Ability to work independently and collaboratively within a multidisciplinary team

Physical Demands
  • Frequent standing, walking, and client interaction
  • Ability to transport clients as needed
  • Use of computers and standard office equipment throughout the workday
