Job description

Position Overview

We are seeking a Bilingual Field Onboarding Specialist to join our team immediately. This is a field-based role focused on conducting in-home onboarding and routine supervisory visits to ensure quality care delivery, caregiver compliance, and patient satisfaction. Once the right candidate is identified, onboarding will begin as quickly as possible.

This role will primarily serve Pima County (including Tucson and surrounding areas), with regular travel required across Cochise, Graham, Greenlee, Santa Cruz, Yuma, and select areas of Gila County. Some visits may be conducted virtually when appropriate based on visit type.


Key Responsibilities

Field Visits & Onboarding

  • Conduct Pre-SOC (Start of Care) in-person orientation visits with new patients and caregivers (approximately 3โ€“4 per week, depending on demand).
  • Perform routine supervisory โ€œOn Careโ€ visits after Start of Care to monitor service delivery, caregiver performance, and patient satisfaction (approximately 5โ€“7 visits per week across a caseload of ~53 patients).
  • Independently schedule and coordinate in-home visits directly with patients and caregivers during daytime hours.
  • Travel to multiple counties as required, including Cochise, Graham, Greenlee, Pima, Santa Cruz, Yuma, and select areas of Gila County.

Supervisory Visit Documentation (Completed Electronically)

Documentation required during each visit:

  • Patient name
  • Date and time of visit
  • Caregiver name
  • Confirmation that care is being delivered according to the care plan
  • Observations related to ADLs (Activities of Daily Living) and IADLs (Instrumental Activities of Daily Living)
  • Patient feedback and satisfaction with caregiver performance
  • Confirmation that services are adequate and meeting patient needs

Qualifications

  • Bilingual in Spanish (Required/Strongly Preferred)
  • At least 1 year of experience in a field-based role involving in-home visits
  • Prior experience working directly with patients and caregivers in a home care setting
  • Demonstrated ability to independently conduct in-home visits and accurately document findings
  • Strong interpersonal skills and ability to build rapport with patients and caregivers
  • Comfortable using electronic documentation tools (tablet-based reporting)

Schedule & Work Environment

  • Visits take place primarily during daytime business hours
  • This is a field-based role with significant local travel
