Job description

Responsibilities
Responsible for being able to perform all aspects of the hospital's assessment and referral process, including efficient and effective handling of inquiry calls, knowledge of and communication with referral sources; providing hospital and program information to individuals, agencies, facilities, and professionals; making outside referrals, when appropriate; assessing and triaging an individual's mental health and/or chemical dependency status; making patient safety and level of care recommendations to the physicians(s); identifying insurance benefits, pre-certification of patient care; completing all admission requirements; communicating patient safety concerns and pertinent patient information to unit nursing staff and documenting all necessary information.
Knows and understands the assessment and referral call system procedure.
Follows Intake call sheet to resolution .
Knows the Hospital services and is able to answer general service information questions, including admission and exclusion criteria.
Completes assessment and evaluation of presenting patients.
Informs patients/families of patient rights, responsibilities, and pertinent rules and regulations prior to admission.
Follows EMTALA regulations and identifies high risk individuals presenting for emergency care.
Effectively communicates information to physician for appropriate orders for services
Initiates verification of insurance coverage to determine viability of insurance for individuals seeking admission.
Performs other duties as assigned/required by this position.
