Job description

The Student Success Coordinator (SSC) reports directly to the Supervisor-Student Success and provides care coordination and psycho-educational support and interventions to students referred to the MindCraft program.  With a caseload of 15 students, the SSC works with students, school-based staff, and caregivers to identify needs and strengths, design and implement interventions to improve the student’s behavioral and/or academic functioning.

Responsibilities:

  • Provide care coordination within a Systems of Care framework to children, youth and families with needs across life domains (social determinants of health) who are multi-systemically involved.
  • Serve as a facilitator, advocate, and liaison, working with individuals and their families to identify strengths and needs.
  • Develop an individualized, comprehensive plan of care.
  • Identify services and resources to support the successful implementation of interventions.
  • Provide individual and/or small group support to students (including behavior management and skill-building interventions).
  • Provide service referrals and connections
  • Identify and address gaps in services and supports which prevent the successful implementation of interventions.

Position Qualifications:

  • Bachelor’s degree in social work, psychology, or a related field required.
  • Minimum of 2-3 years of experience performing the duties outlined above.
  • Minimum of 1-year experience working children and families with intensive needs, knowledge of resources and community supports in Montgomery County, Maryland, and a commitment to working in a fast-paced team environment. 
  • Must be proficient working in virtual environment and comfortable engaging supervisory and team support.
  • Proficient computer/database skills (Microsoft Office), minimum of 2 years of experience working with an electronic health record or similar technology related to service delivery.

Starting salary: $54,000-$56,000
