The
RN: Registered Nurse Case Manager contributes to our Mission of Caring by providing compassionate direct patient care to our clients in a one-on-one setting in their homes. The
RN: Registered Nurse Case Manager conducts nursing visits to include admissions, revisits, and discharges while providing assessments and education to patients. The
RN: Registered Nurse Case Manager for Saint Elizabeth Home Care is a part of a collaborative interdisciplinary team with ample administrative support. Our
RN: Registered Nurse Case Manager is equipped with point of care devices (iPad), as well as all necessary and requested medical equipment and supplies.
Qualifications for a RN: Registered Nurse Case Manager:
- Current unencumbered RN license in Rhode Island.
- Minimum of one year nursing experience, home care experience preferred.
- CPR certification.
- Reliable transportation, current clean driver's license, automobile insurance.
