Lead Patient Safety, Risk Management & High Reliability

At Valley Presbyterian Hospital, patient safety is at the heart of everything we do. We are seeking a strategic and collaborativeDirector, Patient Safety & Risk Managementto lead the organization's patient safety program, strengthen a culture of high reliability, and oversee risk management initiatives that improve patient outcomes and organizational performance. Reporting to the Chief Medical Officer, this leader will partner with executive leadership, physicians, and frontline teams to proactively identify risks, reduce preventable harm, and promote continuous improvement across the organization.

What You'll Do

  • Lead and advance the hospital's comprehensive patient safety and high reliability program.
  • Serve as the organization's Patient Safety Officer and strategic advisor on patient safety initiatives.
  • Oversee patient safety event reporting, investigations, root cause analyses (RCA), failure mode and effects analyses (FMEA), and corrective action planning.
  • Lead executive safety rounds, safety huddles, environmental assessments, and proactive risk identification activities.
  • Analyze safety events, grievances, complaints, and performance data to identify trends and drive system-wide improvements.
  • Partner with physicians, nursing leaders, and operational teams to implement sustainable patient safety solutions.
  • Promote Just Culture, human factors principles, error prevention strategies, and organizational learning.
  • Develop and deliver patient safety education for leaders, physicians, and staff.
  • Lead regulatory reporting, accreditation readiness, survey activities, and patient safety compliance initiatives.
  • Oversee the organization's patient grievance and complaint management processes.
  • Support enterprise risk management, including claims management, litigation support, regulatory investigations, and mitigation planning in collaboration with General Counsel.
  • Develop policies, performance metrics, dashboards, and reports for executive leadership, quality committees, and the Board.
  • Collaborate with Quality, Nursing, Infection Prevention, Compliance, Medical Staff Services, and operational leaders to improve patient safety and organizational performance.

What You'll Bring

  • 7โ€“10 years of nursing experience in a hospital or clinical environment.
  • 3โ€“5 years of progressive healthcare leadership experience.
  • 5โ€“7 years of Patient Safety and/or Risk Management experience.
  • Strong knowledge of patient safety principles, enterprise risk management, high reliability, Lean methodology, and performance improvement.
  • Experience leading Root Cause Analyses (RCA), Failure Mode and Effects Analyses (FMEA), event investigations, and corrective action planning.
  • Experience with regulatory compliance, accreditation standards, and healthcare quality improvement.
  • Excellent analytical, communication, presentation, and leadership skills.
  • Ability to influence organizational change and build collaborative relationships across multidisciplinary teams.
  • Proficiency with Microsoft Office applications, including Excel and Word.

Requirements

  • BSN required.
  • Master's degree required (MSN, MHA, MBA, MPH, JD, or other healthcare-related advanced degree).
  • Current California Registered Nurse (RN) license.
  • CPHRM, CPPS, or equivalent certification required (or obtained within six months of hire).
  • Ability to obtain LA City Fire Card within 30 days of hire.
