This position is located in Clatskanie, OR.
JOB SUMMARY:
The Director of Health Services (DHS) is responsible for overseeing the clinical operations of the Assisted Living and/or Memory Care community while ensuring exceptional resident care, regulatory compliance, and quality outcomes. The DHS provides leadership and support to nursing staff, medication technicians, caregivers, and other clinical team members while promoting a culture of compassion, teamwork, accountability, and person-centered care.
Working closely with the Administrator/Executive Director, the Director of Health Services ensures residents receive individualized care that promotes dignity, independence, and quality of life.
RESPONSIBILITIES:
  • Participate in infection prevention and control activities
  • Ensure medication management practices meet regulatory standards
  • Train, coach, and supervise nursing and clinical staff
  • Provide ongoing education and competency evaluations
  • Conduct performance evaluations and corrective action when appropriate
  • Promote employee engagement, teamwork, and professional development
  • Assist with staffing plans and scheduling to ensure adequate clinical coverage
  • Build trusting relationships with residents and their families
  • Communicate changes in resident condition promptly
  • Participate in care conferences
  • Perform and oversee nurse delegation in accordance with applicable state regulations, including resident assessments, caregiver training, competency validation, and ongoing supervision
  • Address resident and family concerns professionally and compassionately
  • Oversee medication administration systems
  • Ensure medications are ordered, stored, administered, and documented appropriately
  • Collaborate with consulting pharmacists
  • Monitor medication passes and provide coaching as needed
  • Partner with the Administrator/Executive Director to support overall community operations
  • Collaborate with Life Enrichment, Dining Services, Maintenance, Housekeeping, and other departments
  • Foster a positive, resident-centered culture
  • Respond to medical emergencies and critical incidents
  • Participate in emergency preparedness planning and drills
  • Serve as the clinical leader during emergency situations
  • Other duties as assigned

REQUIREMENTS:
  • Registered Nurse (RN) Licensure in the State of practice
  • Minimum of two years of nursing leadership experience preferred
  • Experience in Assisted Living, Memory Care, Skilled Nursing, or Geriatric Care preferred
  • Thorough understanding of state regulations governing Assisted Living and Memory Care communities
  • Strong clinical assessment and critical thinking skills
  • Excellent leadership, communication, and organizational abilities
  • Proficient with electronic health records and Microsoft Office
  • Experience supervising clinical teams
  • Experience with dementia care and memory support programming
  • Knowledge of infection prevention and quality improvement initiatives
  • CPR/BLS certification
  • Must successfully complete a required background check in accordance with applicable federal, state, local laws and organization policy

Located in the tiny town of Clatskanie many of the residents are long time community members. The community has a newly renovated employee breakroom and family like staff.
