J O B D E S C R I P T I O N

Job Classification: High Intensive Care Coordinator

Working Title: High Intensive Care Coordinator – St. Clair County

The High Intensive Care Coordinator position is a direct service provider and an information and referral agent for members of the Target Population (children and adolescents with a serious emotional disturbance and their families who have involvement with multiple child-serving agencies) as assigned by the St. Clair County Coordinator. The High Intensive Care Coordinator will work closely with JBS staff and the child-serving agencies to broker/link services for youth and their families and are in need of mental health treatment and social services linkage using a wraparound informed care practice. The High Intensive Care Coordinator will collaborate with other mental health and social service providers to advocate for the client and family.

PRIMARY JOB FUNCTIONS:

Properly assess strengths and needs and devise an individualized care plan for each client with the assistance of the St. Clair County Coordinator, the family and other care plan members/agencies

Access the target population to a wide range of services in the St. Clair County, to include employment, volunteer, youth group, social/recreational activities as part of the care plan.

Effectively implement the written care plan to meet youth/family goals and objectives.

Serve as a liaison between the youth/family and various service providers to ensure continuity of care.

Provide accurate client information and documentation to the St. Clair County Coordinator and during weekly staffings and to the JBS administrative/clinical supervisory staff as required.

Assist with data collection as required for the Department of Mental Health.

Participate in multiple needs child facilitation team meetings, court reviews, school IEP, child welfare ISP meetings, etc. as an advocate for the youth and family.

Develop and maintain detailed and accurate client records in an electronic medical record system

Provide transportation to youth when necessary.

Perform all administrative duties as required.

MINIMUM QUALIFICATIONS:

Bachelor’s degree in psychology, counseling, social work, or behavioral sciences-related field from an accredited university is required. One year of experience working with children/adolescents with a serious emotional disturbance and their families is required.

Valid Alabama’s driver’s license and driving record acceptable to the agency’s automobile insurance carrier for inclusion on agency’s policy. Suitable automobile to allow for the required travel in this position. Must show and maintain car insurance equal to or in excess of the State of Alabama standards for automobile insurance coverage.

KNOWLEDGE, SKILLS AND ABILITIES:

Knowledge of the special needs and behavioral characteristics of serious emotionally and behaviorally disturbed children.

Knowledge of the use and side effects of psychotropic medications.

Knowledge of the community of St. Clair County and surrounding areas, including available community resources.

Knowledge of the juvenile justice system and how it interfaces with mental health.

Knowledge of the child welfare system and how it interfaces with mental health.

Knowledge of the education system and how it interfaces with mental health.

Knowledge of the rules and regulations governing the human services provider agencies in the Jefferson County area.

Knowledge of the legal and ethical issues relative to confidentiality of client information and records, as well as the ability to interpret and apply legal and ethical standards within the scope of the care coordination position.

Ability to communicate effectively with a community-based, multidisciplinary team.

Knowledge of casework principles and methods related to care coordination.

Working knowledge of interviewing techniques and principles.

Ability and reliability with regard to documenting all client-related activities in a concise and timely manner.

Skilled in interpersonal relationships.

Skilled in organizing and coordinating services.

Effective written and expressive communication skills.
