Outpatient services include behavioral health, nursing, and psychiatric assessment, stabilization and medication maintenance services as well as crisis intervention services as well as awareness, education and recovery groups.

River Edge Behavioral Health is seeking a Full Time Discharge Planner Group Leader to work under the supervision of the Adult Clinical Nurse Manager. The Discharge Planner Group Leader will lead skill training groups to enhance recovery/crisis management skills of behavioral health clients at the Crisis Stabilization Unit. The Discharge Planner Group Leader will act as client advocate to coordinate discharge plans and obtain needed resources for continued recovery after discharge from the Crisis Stabilization Unit. They will complete required documentation to support skill training, assessments as assigned, and resource development for clients.

Duties and Responsibilities
  • Act as client advocate to develop and coordinate available resources to assist in recovery after discharge.

  • Collects information about clients through interviews and observation.

  • Skill training with clients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life.

  • Develop and implement treatment plans based on clinical experience and knowledge.

  • Ability to perform SOS safety checks.

  • Maintain confidentiality of records relating to client's treatment.

  • Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.

  • Prepare and maintain all required treatment records and reports.

  • Ensures safety and security of clients.

Qualifications
  • Bachelor's degree from an accredited college or university in a social service-related field (social work, sociology, psychology, etc.)

  • At least 1 year experience working with behavioral health clients (mental health, substance abuse, developmental disabilities).

  • Certified Peer Specialist (preferred)

  • Provide 2 Professional references.

  • All applicants must pass a satisfactory background clearance and pre-employment drug test.
