Job description

Compensation Range: $16/hr - $18/hr commensurate with experience
Benefits:

Overview:
Case Manager & Outreach Worker at Intercambios Puerto Rico, Inc. with main offices in Fajardo and providing services throughout the eastern region of the island. This position would provide support services to individuals and families of people who are experiencing homelessness, substance use, and/or co-occurring mental health disorders directly in the community (via outreach and community engagement).
Responsibilities:
  1. Provide support services to individuals (and families) who are experiencing homelessness, substance use, and/or co-occurring mental health disorders directly in the community.
  2. Responsible for necessary documentation to create the participant's record: receive, review and file all required documents.
  3. Provide evidence base practices according to the needs of the target population, among these: SBIRT, Motivational Interview, others and document interventions.
  4. Screen individuals (mostly in the community) for recruitment to the GBHI Project.
  5. Complete intake and follow up SUPRT tool.
  6. Maintain an updated register of participants and provide the necessary follow-ups within the established timeframe.
  7. Carry out the processes in compliance with the rules, procedures, and protocols of the organization.
  8. Coordinate and assist in participant transportation services.
  9. Coordinate the medical and social services that participants need
  10. Link program participants to care and support services via referrals and follow up.
  11. Referred and coordinate services with internal and external programs for participants in need.
  12. Document and update the participant record according to the established timeframe.
  13. Provide follow up services to program participants according to the established timeframe.
  14. Coordinate with other team members for participant services or support.
  15. Driving the official or rented vehicle when necessary to complete project tasks.
  16. Identify and locate participants in need of services or who are not receiving services, as referred, and report their status to the team
  17. Prepare monthly reports if necessary.

Requirements:
  • Bachelor's in social work, Behavioral Sciences, or equivalent
  • Up to date driver's license
  • Experience with providing case management services to people that have experienced homelessness, substance use, and/or co-occurring disorders, others.
  • Knowledge about MAT/HIV/AIDS, Hepatitis-C and STDs in the Puerto Rican context
  • Knowledge and/or experience with Evidence Based Practices / Interventions
  • Personal experience with any of the medical and social situations we address in the project (is a plus).
  • Highly motivated to work in an outreach setting. Willing to work in community spaces under changing weather conditions.
  • Proficient and efficient with multiple computer software programs
  • Meticulous at maintaining records and confidentiality protocols.
  • Good written and verbal communications skills (English a plus)
  • Outreach and participant navigation experience
  • Able to work closely with others in outreach team and office setting (team player)
