Community Based Adult Services/Adult Day Health Care facility looking for an Social Worker. Experience in ADHC is a PLUS.


Social Worker Assistant:
Applicant MUST have a Bachelor's Degree in a Social Work, Psychology, Sociology or Social Welfare from an accredited college or university. Exceptions may be made for applicants with relevant job experience who have earned a bachelor's degree in a similar field.
SW Assistant Job Duties:
- Provide assessment and reassessment
- Development and implementation of Individual Plan of Care
- Individual and group counseling to elderly participants as per each individual plan of care
- Planning and coordination of discharge plan
- Documentation of provided services
- Provide various care management services to participants in order to obtain needed services such as IHSS and Access
- Assist with Primary Care Physician appointments
- Provide case management to participants in need of housing referrals and access to social welfare programs
- Complete all relevant paperwork and maintain participants' files
